Position: Case Manager
Hours: M-Th, 9:00-3:00
Salary: Based on qualifications.

Job Description:
Case managers are responsible for case management of the clients. Duties include but are not limited to; client intake and assessment of client needs: determining the type of program/programs that are needed to enable the client a secure and return to self sufficiency.
Responsibilities:
1.) Case managers are responsible for client intake, assessments, distribution of assistance and referrals, and monitoring the progress of each client.
2.) Case managers network with outside agencies and service organizations through telephone communication, meetings, and seminars.
Qualifications:
1.) Bachelor of Social Work or a minimum of 4 years experience in this field.
2.) Excellent computer skills in Microsoft Word, Act, Excel, Outlook, and Publisher. Also, be able to use faxes and copiers.
3.) Must be organized and maintain accurate client documentation regarding interviews, referrals, assistance given.
4.) Must have professional work ethics and habits.
5.) Able to work under limited supervision.
6.) Bilingual very helpful.
7.) Current Texas Drivers License.
8.) Must pass a background check and possible drug screening test.
